Latest Windows Updates Introduce Exchange Server Issues

Organizations utilizing on-premises Microsoft Exchange Server have recently encountered unforeseen stability challenges following the installation of the October 2026 Windows Server updates. Microsoft has confirmed that these updates may lead to unreliable Exchange services in specific environments, particularly those managing heavier workloads. Reports from administrators indicate a range of issues, from connectivity failures with Outlook on the web to significant disruptions in mail flow, prompting IT teams to urgently seek the underlying cause.

The impact of this issue has been notable, with email queues swelling, inconsistent user access to messaging services, and some environments facing database availability group (DAG) complications that hindered service continuity. These symptoms typically emerged post-update, causing IT administrators to devote substantial time to troubleshooting Exchange rather than addressing the Windows Server component responsible for the instability. This bug affects Exchange Servers operating on Windows Server versions 2016, 2019, 2022, and 2025.

A registry-based mitigation is available to fix Exchange Server issues

In response to these challenges, Microsoft has provided guidance indicating that the root of the issue lies in limitations related to Endpoint Mapper requests. The company has recommended implementing a registry-based mitigation aimed at enhancing capacity and minimizing the chances of service degradation. While Microsoft continues its investigation, it is also working towards a more sustainable solution.

“For workloads such as Exchange Server, the default limit of 512 simultaneous Endpoint Lookup calls might not be sufficient under heavy load. We are investigating this issue and will provide more information when it is available,” Microsoft noted on the Windows Release Health Dashboard.

This situation underscores a prevalent challenge faced by IT teams: the delicate balance between maintaining security and ensuring stability. Organizations are advised to uphold critical security updates to remain protected, yet doing so may necessitate additional mitigation measures and closer monitoring of affected systems until a long-term resolution is achieved. The incident serves as a poignant reminder that even routine patch deployments can introduce operational risks that must be carefully managed alongside security considerations.
